We have developed customized Implementation Forms for each college. You can find them on the Implementation Form page.

They have been prepopulated with data where possible. Please verify you have the correct form after you have downloaded it.

The completed forms are due on October 15 and should be sent to lgiovando@ocls.ca AND Maria.Bunevski@exlibrisgroup.com.
